Surgery Disease
Omphalocele:
Intestines coming out through unbiblical cord and of intestines and liver have been observed in 1.5000 and 1.10000 newborns respectively.
During superficial review and examination of an infant, Omphalocele is completely obvious. A layer that covers come out guts of abdomen is limpid and lack of veins and umbilical cord has been seen following it. (Coming out of abdomen guts in a part that umbilical cord is joined to abdomen).
During fetus period within 10 weeks of life in uterus, if intestines and guts don’t return to abdomen, it will cause Omphalocele. Abdomen is relatively small because its contents result in increasing the size of abdomen cavity.
Treatment:
Treatment is done with surgery (replacing guts and closing abdomen wall) that will be done during several stages.
Nursing:
Using an incubator is the basis of taking care of affected child.
Putting NGT of abdomen suction to prevent of flatulence of abdomen and intestine. Preventing of any pressure on abdomen guts and preventing of its rupture.
Keeping guts warm and damp and prevention of its pollution by suitable cover(this cover includes wet and clean gauze with normal saline .Then put a dry and clean towel on it and cover it by plastic).
Entering the contents into abdomen cavity gradually and calmly (to prevent of pressure on diaphragm and down front vein and respiratory disorder and smelling of down organs).
Respiratory control (suitable respiratory way, changing the position inhibits telecasts and disorder because of guts pressure).
Controlling down organs regarding swelling (because of guts pressure on down front vein).
Nutrition after surgical operation is done through peripheral vein and gastrostomy respectively. Glucose and then milk are added gradually.
A child's appearance affected to omphalocele may be unbearable emotionally for parents. A nurse should inform them about the fact when she supports them to be able to meet eagerly a child's needs and improve her emotional relation with an infant.
