1. Is the presence of father necessary in hospital for a child's operation?
Yes, his presence and satisfaction are necessary. Every body can give satisfaction for himself/herself if their age is above 18 but children who have not reached the legal ago, their legal guardian should give the satisfaction.
2. If a patient's father died and he/she hasn’t reached the legal ago, can his/her mother sign the satisfaction form?
In this case, a patient’s grandfather can sign the satisfaction form. A mother can't sign the satisfaction form and legal guardian should be determined for a patient through inquiry from legal references.
3. What should one do for taking therapeutic background of a patient?
By referring to hospital, writing demand and chief’s agreement of hospital, the copy of case report including tests, answer of sonography, radiology and biopsy, description of operation and summary of case are given to a patient and /or his/her legal guardian. The reason of a reject would be determined.
4. What equipments are there for a child affected to cancer?
all therapeutic pieces of hospitalized and flying patient are paid by institute of protecting children affected to cancer to his/her family. Necessary documents for delivery to institute and organizing a case for a patient are as follows:
A copy or print of account list, biopsy, aspiration of marrow of bone, certificate of a physician based on type of disease and confirmation of treating physician and sending one formal letter for institute from hospital which shows the dates of reception and discharge and the paid sum by a patient have been written in. Flying prices of a patient are given to unit of tariff by providing its paid receipt and separate letter for flying prices is given to a patient.

5. What documents are necessary for the use of insurance from Imdad committee?
Card of therapy, taking form of reference from Imdad committee.
6. If a patient isn’t insured, is it possible her/his being insured?
Urban hospitalization insurance can are used and necessary documents for being insured are as follows:
- Identification of all members in patient’s family.
- Copy of first and second pages of patient’s father.
- Copy of first pages of identification of other members of patient’s family.
7. What should we do for using rural insurance?
After referring to a village's council and its confirmation, you should refer to insurance office located on 8th Reza, first 30 meters of Ahmad Abad and then after their confirmation, a card of rural insurance will be issued.
8. What should we do for using a card of rural insurance for a patient?
Providing following documents is necessary:
- A form of reference from a center of villages’ hygiene with its copy.
- A copy of first, second, third pages of patient’s parents’ identification.
- A copy of first and last pages of a patient’s identification.
- A patient’s rural insurance card and its copy.
- A patient’s parents' rural insurance card and its copy.
- A pictured rural insurance card for children above two years old is essential.
9. What documents are necessary for discharge of a patient?
Having the following documents is necessary during discharge:
-Two copies of first page of a patient’s insurance notebook for insurances of army, social providing and therapeutic services (other ranks, urban and hospitalized patients).- Providing letter of introduction of a bank with its copy for the insured of banks.
-Having two copies from first page of a patient’s identification.
